Managing the patient with intractable asthma

Abstract

Patients first should be evaluated for a mimicking or coexisting condition, such as vocal cord dysfunction or endobronchial stenosis. Once the diagnosis is clearly established and any triggering factors have been addressed, chronotherapy is the key to management--that is, timing drug delivery so that serum levels peak when disease manifestations are at their worst, often in the middle of the night.
