Spontaneous recurrent seizures mediated cardiac dysfunction via mTOR pathway upregulation: A putative target for SUDEP management

Abstract

Alteration in electrophysiology leading to cardiac dysfunction is a major complication in patients with epilepsy that leads to life threatening condition known as Sudden Unexpected Death in Epilepsy (SUDEP). The present study was designed to understand the biomolecular changes and cardiac parameters during different phases of epileptogenesis in lithium-pilocarpine (Li-pilo) rat model of epilepsy. The animals were exposed to Li-pilo to induce spontaneous recurrent seizures (SRS). Non-invasive blood pressure and electrocardiography was recorded at 7th, 28th and 75th day following pilocarpine administration; considered as latent, initial SRS and late SRS phases, respectively. Serum biochemistry, cardiac histopathology, protein and mRNA expression levels were studied following electrocardiography on day 75. Mean arterial pressure was found to be decreased in latent phase, whereas showed progressive increase during initial and late SRS phases. Prolonged QTc interval was observed during the late SRS phase as compared to that of basal and latent phases.
