The dilemma of managing human rabies encephalitis

Abstract

Human rabies acquired from dogs and other terrestrial mammals remains 100% lethal. Two people bitten by bats in the USA have recovered from rabies encephalitis to live an independent life.(1,2) Many other rabies patients treated with expert intensive care have died in the acute phase or been left with severe neurological impairment. With such a record and no available specific antiviral treatment, how should clinicians manage patients with this appalling disease, particularly in poorer countries where dog rabies is endemic?
